Referrals
It is our policy to treat all illnesses and injuries as indicated. Referrals are given at the discretion of the primary care provider. If you are referred to our clinic, be sure and verify with your insurance company that the provider is covered under your plan. If your plan requires you to have a referral prior to being seen by one of our providers, it is your responsibility to obtain the referral or we may not be able to treat you.
Payment Terms
Your insurance requires that you pay your co-pays at the time of service. If you do not have health insurance, we offer a prompt pay discount, provided you pay in full at the time of service.
If you are going to have surgery, we require you to meet with our financial specialists who will determine your portion of the bill and assist you in making payment arrangements. We realize you expect us to provide you with quality healthcare and we will do this, in return, we ask that you honor your financial obligations to us and pay your portion prior to the surgery.

Telephone calls
We strive to return all telephone calls by the end of the business day. If your call comes in after 4:30pm, it may not be returned until the following day. If the physician is out of the office, we may suggest you contact the physician on call who is better qualified to manage your concern.
Patient Rights
Patients have a Bill of Rights that is posted in our lobby. Patients who refuse to follow medical advice or exhibit unacceptable behavior may be dismissed from our practice. This is at the discretion of the physician.
