Q: What insurances do you accept?

A: Our office accepts most insurance plans. Please call if you are not sure and we will be glad to assist you. Please bring your insurance card and a picture ID with you to your appointment.

Q: How do I refill my prescription?

A: When you need a prescription refill, we request that you give us 48 hours notice. Please do not wait until you run out of medication. Please phone your pharmacy for refill requests. If your prescription has run out of pre-authorized refills, they will fax a request to the office. This is the most efficient way to receive your refill medications. Occasionally, your physician may not automatically authorize a refill and will require you to make an appointment to be seen.

Q: How can I get medical help after hours?

A: If you require a physician, after hours, we have an on-call physician available. The on-call physician can be reached by calling the Physician Answering Service and have the service page the physician on call. The number to call is 1.877.412.2377. If you have a life-threatening emergency, please dial 911.

Q: How can I get a copy of my medical records?

A: Requests for copies of your records require your written authorization. All record requests will be processed within 30 days from the request. We forward applicable copies of your records to any physician we may refer you to at no charge to you. If you request additional copies, there may be a charge for these records.
